Clinic Construction

Prism Health is undergoing the last of a 2–phased remodel to accommodate our expanding services. From March 6 through April 11, the Belmont building will be closed to in-person primary care, laboratory services, and Pivot testing services from 8:30 a.m. to 2:00 p.m., Monday through Friday.  

The clinic will open to in-person, pre-schedule appointments each day from 2:00 p.m. to 5:00 p.m. through the construction period. 

Behavioral health services will only be available by telehealth during construction. 

Avita pharmacy will maintain normal business hours through the construction period: Monday-Friday 9:00 a.m. to 5:30 p.m. 

We will do our best to meet the needs of our patients during this time. However, patients may experience slightly longer call back times. Please see below for some answers to questions you may have about services during the March 6-April 11 construction period. 

  • Telehealth services are available for both primary and behavioral health care from 8:30 a.m. to 5:00 p.m., Monday through Friday. However, in-person services for primary care are limited to 2:00 p.m. to 5:00 p.m. during the construction period. Appointment availability will be limited during this time. If your medical need is urgent, you may be asked to go to urgent care.

  • You can schedule using MyChart. If you are not a MyChart user, call our main line during business hours to schedule: 503-445-7699.

  • Prism health’s lab will be open from 2:00 p.m. to 5:00 p.m., Monday through Friday during the construction period. If those times do not work for you, medical staff will direct you to a Quest Diagnostic location nearest you.

  • Call your pharmacy to submit a refill request or submit your request through MyChart. Please allow 3 business days for any medication refill request.

  • Avita pharmacy will remain open during construction. Their hours of operation are Monday-Friday 9:00 a.m. to 5:30 p.m.

  • Pivot HIV and STI screening services will be conducted out of our Davis Street location on Mondays during the construction period. Tuesday and Saturday services will maintain normal hours at Prism’s Belmont clinic. Call 971-279-7033 for more information or to schedule.

  • If you have a medical question or concern and need to reach out to someone on your care team, please send a message though MyChart.

  • Prism Health has a 24/7 nurse-advice line. If Prism is closed, call our main line at 503-445-7699 and follow the prompts to speak to the after-hours advice nurse.

    If you have a medical emergency, call 9-1-1 or go to the nearest emergency department.

    If you have a mental health emergency and Prism is closed, please call the Multnomah County Crisis Line at 503-988-4888 or dial 9-8-8 for the national Suicide and Crisis Lifeline.
