Prism Health FAQ’s

  • Prism Health provides high quality, affordable, and accessible health care for all, with a focus on serving individuals who identify as lesbian, gay, bisexual, transgender, queer, plus all other gender and sexual minorities (LGBTQ+). All Prism services aim to reach the most vulnerable – those living below the poverty line, communities of color, houseless or unstably housed individuals, and people experiencing mental health and/or addiction issues. Prism Health is a program of Cascade AIDS Project (CAP).

  • We opened Prism Health – Belmont in 2017. The Belmont clinic is our first integrated primary care and behavioral health location. Come visit us in person at:

  • To serve more patients, we will be opening Prism Health | Morris, our second integrated primary care and behavioral health clinic, in Summer 2024. Come visit us in person later this year at:

  • FQHCs aim to provide quality healthcare to medically underserved patients. This means providing services to patients who otherwise may not have access to affordable care due to financial hardship. As an FQHC we have a sliding fee scale for our patients and no one will be turned away due to an inability to pay.

  • Pivot is dedicated to providing free HIV and STI screenings to the LGBTQIA+ community by appointment. Prism Health provides comprehensive on-site testing for HIV and other STIs through either your primary care visit or our Pivot services.

  • Not all pharmacies are created equal. Free delivery, expert staff, online refills- theses are the basics you should expect from your pharmacy. Avita has the basics covered.

  • Prism Health provides high quality, affordable, and accessible healthcare services including Primary Care, Behavioral Health, & Pharmacy Services. Our integrated approach to healthcare helps us meet the diverse needs of our patient community. We see patients who need routine healthcare check-ups, sexually transmitted infection (STI) testing or treatment, cancer screenings, vaccinations, HIV care, PrEP& PEP navigation, individual therapy, group therapy, couples & family therapy, substance use support, gender-affirming services, pharmacy services, and more!

  • Prism Health strives to provide compassionate, affirming primary care for transgender and gender non-conforming folks in a safe, friendly, and culturally responsive atmosphere.

    Our patient-directed, trauma-informed approach to care provides for a variety of affirming healthcare needs including:

    • hormone therapy, education, and management

    • referrals to gender affirming surgeries

    • coordination with behavior healthcare providers to provide mental health letters of support based on WPATH standards of care for gender affirming surgeries

    • gender-affirming pap smears, pelvic exams, and prostate exams

    • chest health

  • Prism offers the full spectrum of care for those living with HIV. Our providers get to know you as the patient so that we may empower you to make the best decisions for your wellness. We combine evidence-based medicine with clinical experience and patient preferences to keep you healthy. Our goals are to get you to undetectable, keep you there, prevent transmission, and prevent other diseases. We take pride in caring for the diverse community affected by HIV.

    We provide:

    • Medication management

    • Lab testing on site

    • Treatment of co-infection with Hepatitis B and C

    • Vaccinations

    • Contraception

    • Gender-affirming care

    • Mental health care

    • Cancer screening

    As part of the Cascade AIDS Project, we are able connect you to services to help support housing, access to food, insurance navigation, case management, and peer support. See more at capnw.org.

  • We know that seeking support for substance use can be daunting, and overwhelming. At Prism, our substance use counseling and groups are centered on harm-reduction, that may or may not include abstinence. Our services are provided by a dual-diagnosed clinician, meaning your substance use and mental health can be managed in one place. We currently offer 1-to-1 counseling and support groups (Seeking Safety, Living in Balance, and a process group). Other support groups are also available .

  • To get started with Prism Health, please complete our To get started with Prism Health, please complete our Establish Care Request Form. The information you provide will be reviewed to ensure that your health insurance is one that Prism Health accepts or, if you are uninsured, to connect you with a enrollment navigator who can help determine if you are eligible for the Oregon Health Plan or our Sliding Fee Discount Program. Then, an appointment scheduler will reach out to you when a new patient appointment is available.. The information you provide will be reviewed to ensure that your health insurance is one that Prism Health accepts or, if you are uninsured, to connect you with a enrollment navigator who can help determine if you are eligible for the Oregon Health Plan or our Sliding Fee Discount Program.   • Yes! When you complete the Establish Care Request Form you can select your preferred location.

  • You do not have to live in Portland to be a patient at Prism Health.

  • Yes, at this time, you do have to live in Oregon to be a patient at Prism Health.

  • Yes, Prism Health provides telehealth for patients living in Oregon.

  • Prism Health currently sees folks aged 15 and up.

  • We accept several group health insurance plans including Medicaid and Medicare.

  • Prism Health can connect you with an insurance/benefits navigator through CAP. They can determine if you are eligible for OHP or other benefits, or refer you to a member of the Prism Health team who can help determine your eligibility for Prism’s Sliding Fee Discount Program (more info in next FAQ below).

  • Yes. Prism Health offers a Sliding Fee Discount Program to those that fall at or below 200% of the Federal Poverty Guideline. This is determined primarily by the number of folks living in your household and household income or other financial benefits. We will ask for proof of income or a statement of no income. Anyone can apply for the Sliding Fee Discount Program. If approved, services at Prism, our partner pharmacy Avita, and diagnostic laboratory expenses through our partner Quest, are greatly discounted for one year.

    • There is free street parking near the Belmont clinic.

    • There is a handicap accessible parking space on the SE 23rd just south of SE Belmont Street.

    • Prism Health | Belmont is accessible by TriMet bus line 15 Belmont/NW 23rd.

    • There is a small parking lot on the north side of the clinic, and additional free street parking near the clinic.

    • Prism Health | Morris is accessible by multiple TriMet bus lines 4, 6, 24, and 44

    • CAP is always in need of volunteers to lend a hand. If you are interested in becoming a CAP volunteer, email volunteer@capnw.org

    • Prism Health patients are often struggling to make ends meet and pay for basic needs that impact their ability to focus on their health. Our patients often experience food and/or housing insecurity, a need for utility assistance, transportation assistance, and more. These are called Social Drivers of Health. As an FQHC Lookalike, Prism Health receives no federal funding, which makes it difficult for us to help patients with these basic life needs. Monetary donations to help support our patients' basic needs is greatly needed and greatly appreciated.
